ATHLETIC and ACTIVITIES PARTICIPATION DIRECTIONS:
Mid-Del Schools would like to thank you for taking part in our new process of completing athletic and activities participation forms online. This new automated process will allow us to be more efficient in handling the forms, as well as saving valuable resources. You will be able to read through all the policies via the drop down menu labeled “Mid-­-Del Policies” or via the “Athletic and Activities Consent and Signature page. “

• Both the student and parents will sign the Athletic and Activities Consent and Signature page (AACSP). When you sign the Athletic and Activities Consent and Signature (AACSP)” page you will be signing for all documents listed on that page.

• Everyone participating in any extracurricular activity MUST complete the following before they can begin any participation.

• To access the online forms hold your curser over (or tap) the “Electronic Participation Forms” tab. You will then see a drop down list with “athletic and activities consent and signature page”.

• You must have your student’s ID number available. Your Student ID number is not the same as your lunch number. To get you student ID number, call the office at your school.

• To sign the document, Click inside the signature box and hold your mouse over the box. This will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click the “Refresh” icon next to the signature box.

• Once you have filled out all the information on the page and hit submit, it will be delivered electronically. You will receive a confirmation email for each form when the document has been reviewed and accepted by our Mid-Del staff. If you do not get an email after completing the form please try again and resubmit the form. If you continue to have problems please call or email one of the people listed below.

Turn completed physical form into the Athletic Office at the school your child will be attending.
